General description:

The project will be carried out in order to learn how to make a correct clinical history, respecting their vital structure and utility, with a main focus on respect and full exercise of the rights of patients, forgotten today in most and clinics have meant misdiagnosis or indifference to various treatments. That’s why this project besides knowing searching process for an objective clinical history, promote compliance with those rights, and to ensure a good doctor-patient relationship, based on trust, a correct diagnosis and effective treatment, in addition to promoting good decision-making focused on human rights regardless of the situation that they are facing.
The talk will be delivered by experienced doctors and experts in the field. The results will be evaluated by a comparative analysis of questions answered before the talk and then talk, to know what the impact of the project on medical students.

Problem statement:

According to art. 6 of the Code of Medical Ethics: “The doctor since he is called to attend to a sick person, is responsible for providing all necessary medical care to regain its health. His major responsibility is the preservation of the life of the patient”. According to art. 25: “The doctor has an unwavering obligation to respect the principles enshrined in the Declaration of Human Rights. Their professional practice will be governed by these principles which could not be violated in any case be this civil, criminal, political or national emergency”.
Unfortunately, the reality that exists in Ecuador is another, many doctors are unaware of human and patient’s rights, the ones that are violated every day. The lack of knowledge regarding the topic has led to a misdiagnosis, a poor doctor-patient relationship and a detachment of the patients to their medical treatments. This problem has led to serious situations; in a study in the emergency room of General Hospital Enrique Garcés in Quito 95% of health professionals have suffered verbal or physical violence from patients or their relatives (Coello, 2017). This is the reason why we created this project, believing that a conscious formation of our future doctors will lead to a better doctor- patient relationship, in which we would promote empathy, solidarity and respect towards the patients and the rights they have.

Target groups and beneficiaries:

Active members of AEMPPI-Ecuador and OIECS PUCE (OL).
With the completion of this project benefited the long term are the patients themselves, because they deserve that their rights are met and promoted around the world to improve their well-being and quality of life from optimal medical care quality and guarantees they are heard and cared for in the best way, to thereby generate good diagnostics, more effective treatment and a good doctor-patient relationship. In the same way this project is aimed to students who are taught basic skills and techniques for conducting a good history ensuring the benefits mentioned above.

Objectives and indicators of success:

General objective (SMART): To strength the knowledge of 50 medical students and to know the importance of making a good clinical record respecting and promoting the rights of the patient in order to obtain a good diagnosis, by health professionals, through a project of theoretical-practical nature of approximately 5 hours, which will be evaluated by feedback at the end of the event.
Indicator: minimum attendance of 40 students; positive feedback post-event (approximately 85% of acquired knowledge).
Specific objectives:
• Establish knowledge to improve the doctor-patient relationship.
o Indicator: positive feedback post-event (85%) (the acquired knowledge and practical skills will be evaluated)
• Analyze duties and rights of the patient and physician to achieve continuous practice of these.
o Indicator: positive feedback post-event (85%) (acquired knowledge measured)
• Knowing the importance of good clinical history to obtain a diagnosis.
o Indicator: positive feedback post-event (85%) (acquired knowledge measured)
• Clarify doubts about it.
o Indicator: Satisfaction of at least 80% by attendees about the event (the development and impact of it will be evaluated).


3 hour talk that will be dictated by expert doctors on the subject.
The topics that will be taught are:
• Introduction Clinical History: doctor-patient relationship:
• basic structure of the medical history and approach to patient rights and current skills.
• Importance of the Clinical Record focused on the patient.
• Analysis: Ethics and Health.
• Medical duties and rights.
• Space to answer questions.
• Discussion of related clinical cases.
In the same way, the practical phase will be fulfilled with a methodology of simulated patients in which the students develop the Clinical History and apply what they have learned:
• Division of doctor-patient roles: simulated patient methodology.
• Development of the practice based on what has been learned.
• Discussion about the experience.

Plans for evaluation:

• Before the activity: They will be evaluated with questions (using Google forms) short on the subject to try to assess prior knowledge that will be compared at the end of the project.
• After the activity: the results will be evaluated by a test that will be on the platform forms google plus feedback about the event to assess impact and attendee satisfaction.
